Procedure: Endonasal Rhinoplasty (Scarless Nose Surgery) with Chin Implant and Buccal Fat Removal
Goal: In this case, endonasal rhinoplasty alone was not sufficient to achieve the desired outcome. Following a detailed facial analysis guided by the Golden Ratio, Dr. Escobar, Fellow Member of the American Academy of Facial Plastic Surgery (AAFPRS) determined that complementary procedures were required to meet the patient’s goals: a chin implant to enhance projection and facial balance, and a conservative buccal fat removal (bichectomy) to refine the lower third of the face.
Patient presentation included:
- Underprojected nasal tip with wide bony dorsum
- Thick skin with tip asymmetry affecting nasal definition
- Weak septal support compromising function and aesthetics
- Excess buccal fat volume, blunting the jawline and cheek contour
- Micrognathia (when the chin sits further back than normal compared to the rest of the face).
Outcome:
Through a closed rhinoplasty (scarless), the nose was refined, aligned, and structurally supported. The addition of a chin implant enhanced jawline proportion, while buccal fat removal improved definition of the lower third of the face. Together, these procedures harmonized the patient’s profile, corrected nasal and chin imbalance, and delivered a naturally balanced result consistent with her unique ethnic features.
