Before & After Results: Male Revision Rhinoplasty with Thin Skin (Closed Approach)
Procedure: Closed revision rhinoplasty in men after two previous open approaches, correction of structural deformities, and removal of a problematic graft.

This patient had undergone two previous rhinoplasties, both through an open approach. These procedures left him with significant anatomical distortion of the nasal tip, leading to both aesthetic deformity and functional compromise due to nasal pinching, a condition that narrows the airway and obstructs breathing.
In this case of revision rhinoplasty in men the greatest concern, however, was localized at the tip skin. A previously placed oversized Sheen graft, combined with the patient’s naturally thin skin, was progressively damaging the overlying tissue. The pressure exerted by the graft edges threatened to erode through the skin, jeopardizing both appearance and function.
Dr. Escobar performed a closed revision rhinoplasty in men using his Modular Technique, an evolution of the Combined Auricular Graft method he has published. This revision rhinoplasty in men, included careful removal of the problematic graft, reconstruction of the tip framework with auricular cartilage, and the use of the patient’s own fibro-fatty tissue to reinforce and protect the delicate nasal tip skin. This approach not only restored structural support and improved airway function but also optimized healing by preserving blood supply and preventing further thinning of the tip skin.
